Processing of the personal information

This is to represent that I accept the processing of my personal data by BRE Bank SA with its seat in Warsaw, ul. Senatorska 18, as Administrator of personal data in the sense of the provisions of the Personal Data Protection Law dated 29 August 1997 (consolidated text: Journal of Laws from 2002, No. 101, item 926, as amended) for purposes of promotion and marketing, in particular for purposes related to the operation of the BRE Bank Group internet press centre, and the submission of commercial information in the sense of the Electronic Provision of Services Law dated 18 July 2002 (Journal of Laws from 2002, No. 144, item 1204) by the BRE Bank Group which comprises companies with equity links with BRE Bank SA (BRE Leasing Sp z o. o.; BRE Corporate Finance S.A.; Dom Inwestycyjny BRE Banku S.A.; SKARBIEC Asset Management Holding S.A.; SKARBIEC Serwis Finansowy Sp z o. o.; SKARBIEC Towarzystwo Funduszy Inwestycyjnych S.A.; Powszechne Towarzystwo Emerytalne Skarbiec-Emerytura S.A.; SKARBIEC Investment Management S.A.; BRE Agent Transferowy Sp. z o.o., BRE Faktoring, emFinanse Sp. z o.o., BRE Ubezpieczenia Sp. z o.o.).

I represent that I am aware of my rights under the provisions of the Personal Data Protection Law, i.e.:

• the voluntary provision of my personal data;
• the right of access to my data and the right to modify the data;
• the right to object to the processing of my data for purposes of promotion and marketing of the services and products of the companies of the BRE Bank Group mentioned above.
